Angel home care is looking for an experienced care coordinator to join our team.
Hours: 40 hours a week – 5 days out of 7-day contract flexible to meet the needs of the Business.
This is an extremely important role within the organisation as the effectiveness of the care coordination impacts the whole delivery of the service. The care coordinator is responsible for the continuity of care for all our customers. This vital role has a huge impact on the lives of many people who rely on our care and support.
Duties and responsibilities • Dealing with day-to-day phone calls from service users, families, Social workers, carers.Provide out of hour on-call support.
Rostering • Managing the weekly rota by matching the right carers and service users together for the best possible outcomes.
Supporting operations to deliver a good service
Carrying our initial assessment of for new referrals
Carrying out risk assessment, care reviews, spot checks, staff supervision meetings.
Good understanding of safeguarding for adult/children and the channels of reporting.
Ensure high standard of care is always delivered across the board.
Carry out internal audit and ensure services are following all CQC regulations.
Desirable • Computer experience and proficient in Microsoft office applications
• Flexible and reliable
Driver with clean driving licence and own car.
Previous experience is necessary.
