Are you the person we are looking for to facilitate person centred practice? You have the opportunity to be part of Creative Support’s multidisciplinary Complex Care Team. This is a new and exciting role, it is a varied and will require flexibility. You will spend time with the people we support and the key people in their lives to identify their goals and aspirations and work with teams to achieve these within their everyday support.
You will have the opportunity to work alongside our managers and Complex Care Team to contribute to real outcomes based on genuine values of enhancing quality of life to embed person centred practice.
As Practice Facilitator you will be based at our Head Office in Stockport and support our services across the region and nationally. You will be supported within your role and have an opportunity to work with skilled practitioners who are genuinely motivated by the people we support. We will work together to ensure a focus on the person, their qualities and strengths, and provide support that makes a real difference in people’s lives.
Choice
Creative Support is a non-profit provider of person centred care and support to people with a range of needs including older adults, people with learning disabilities, mental health needs and autism spectrum conditions. You must be able to engage with the individuals we support, their families and multi-disciplinary teams to facilitate our Opportunity, Choice and Wellbeing reviews.
This role is innovative and dynamic with a focus on identifying what is important to each person based on our key indicators of quality of life. You will need a creative approach to exploring aspirations and an ability to turn these into practical goals and plans that are supportive to the person and their teams. A key aspect of the role is to motivate staff and ensure that support is driven by these goals, celebrating progress and achievements.
You must be committed, professional, and have experience with working with the range of people we support and a related qualification. You will need to be an excellent and thoughtful communicator with the ability to sensitively facilitate meetings. You will have skills in hands-on support to role model, coach teams and enhance our commitment to person centred practice and positive behaviour support. It requires you to be personally driven and have the qualities needed to inspire others and fulfil the role.
