Stockport Crisis Response Team are pleased to be able to offer a permanent full time post for a Senior Support Worker.
We are a proactive and dynamic community team, working across the 8 Neighbourhoods of Stockport to prevent avoidable hospital admissions and ED attendances. You will be expected to drive and have access to a vehicle.
You will be part of a multi professional team, undertaking higher level nursing, therapy and social care tasks within agreed parameters, delegated by the registered practitioners. The service aims to prevent unnecessary hospital admissions by supporting people in a crisis situation to remain in their own homes and by facilitating early and supported discharge from the emergency department of Stepping Hill Hospital. You will be involved in all aspects of the assessment, treatment and discharge process, and work alongside both health and social care colleagues to ensure that patients remain as independent as possible and achieve the best possible health and social outcomes.
This is a challenging yet rewarding role in a fast paced environment which will give you the opportunity to utilise your senior support worker skills.

If successful this vacancy will require an enhanced DBS check at a cost of £43.50. This cost will be deducted from your first Trust salary
In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
